All minor riders under the age of 18 attending an event and/or practice Sweney Cycle Ranch must have a parent present or an appointed guardian/authorized adult with notarized statement from parents or legal guardian giving permission for another adult (this includes step-parents, brothers, sisters, grandparents, aunts, uncles, cousins, friends, etc.) to act as authorized adult indicating they have been given the authority to be responsible for the minor and to be able to sign releases for minor in order to compete, practice or participate in any activities at Sweney Cycle Ranch. The parents, legal guardian or authorized adult must remain present on the premises while the minor in question he/she is responsible for is at the event. All person with minor signing any release must be prepared to show proof of identity and guardianship. Please note: You may want to have (2) Temporary Guardianship Forms (notarized) as we will be keeping an original form to attach to waivers. If you are a guardian/authorized adult, please present your notarized document at registration. All of the forms can be found below on this page!

This form is REQUIRED for any rider that is 17yr or younger and will be attending a practice or event without their parents or legal guardian. This form must be printed with both page 1 & page 2 on one piece of paper (front & back). This form cannot be accepted if pages are stapled. This form needs both front & back and notarized.
This form is recommended if a rider that is 17yr or younger will be attending a practice or event without their parents or legal guardian. This form will help expedite emergency care for your child. This form will be used in conjunction with the Parental Consent Form.
